Ending Mileage contest form

I am needing an ending mileage contest form. Can someone direct me in the correction direction where I can downloaod a form so I can fill it out.

Is this for 2011? Since the mileage contest just started and doesn't end until October, I don't think the stop form is available yet.

You might want to call the office and ask. I can only find the start form on-line.

Yeah... I got a new bike and wanted to submit the miles to old bike when I fill out new form to miles of new bike but guess I can wait...

I purchased a new bike in the middle of the mileage contest... I need a beginning mileage contest form and not finding it to download on the website... is anyone able to point me in the right direction?
